To participate in Tracks 2 and 3, teams are required to register, sign the n2c2 Rules of Conduct, and sign Data Use Agreements for MIMIC (Track 2 Subtasks A–C, Track 3) and for the University of Washington (Track 2 Subtasks B and C). See below for details on each DUA.

## MIMIC DUA (Track 2 Subtasks A–C, Track 3)

You must assemble and submit a PDF demonstrating that you have both of the following:

- A credentialed account on the PhysioNet website
- An approved DUA associated with the MIMIC-III Clinical Database (v1.4)

**In 2019, PhysioNet changed their credentialing system, and the profile views differ for pre-2019 and post-2019 PhysioNet accounts.** Where applicable, the instructions below differentiate between these account vintages.

### Steps

1. Log in to **PhysioNet** with your email and password.
2. On the top right corner, you will see your profile name with a dropdown menu.
3. Go into the **Settings** page.
4. Capture credentialing information:
    1. If you were credentialed **pre-2019,** go to **Emails** and capture a screenshot that includes both your **username** and **email address** to include in a PDF for upload.

    2. Then go to **Credentialing** and capture a screenshot that includes both your **username** and the **credentialing date** to include in a PDF for upload.

    3. If you were credentialed **post-2019,** your **Credentialing** page will include a **View my applications** link, which will open your My Credentialing Applications page. Take a screenshot of the **My Credentialing Applications** page to include in a PDF for upload.

5. Return to the **Settings** page, then **Agreements.**  
    If you were credentialed **pre-2019,** the Agreements page may be empty. To add MIMIC-III, perform the following:
    1. Go to: <https://physionet.org/content/mimiciii/1.4/>
    2. Click “sign the data use agreement” at the bottom of the screen
    3. (Re)submit the MIMIC-III agreement
    4. Return to the Agreements page and confirm MIMIC-III is included

6. Locate the **MIMIC-III Clinical Database (v4)** in the Projects column and click on **View** corresponding to it.
7. A new tab opens with the MIMIC-III DUA signed for the dataset. Take a screenshot of the page to include in a PDF for upload.

8. Create a PDF of your screenshots to show:
    1. Your credentialed account on the PhysioNet website
    2. Your approved DUA associated with the MIMIC-III Clinical Database (v1.4)

After your MIMIC access has been confirmed *and your entire team has been finalized and approved for Track 2,* you will be able to download the Social History Annotation Corpus (SHAC) Guidelines immediately, and to participate in Subtask A (Track 2A): Extraction.

For your team to participate in Subtasks B and/or C (Track 2B and/or Track 2C), continue through the steps of completing the UW DUA.

## University of Washington DUA (Track 2 Subtasks B and C)

Access to University of Washington (UW) data requires completion of the UW DUA. Download the PDF and complete pages 1 and 5. You may either a) use Adobe Acrobat to complete/sign the form electronically or b) print a hard-copy, complete/sign it by hand, and scan it. Be sure you have completed all of the following items:

1. Your name in the **Recipient** space at the top of page 1
2. Your **Signature** on page 5
3. Your **Name** on page 5
4. Your **Title** on page 5 (this is your ***job title*** such as Associate Professor or Student, *not* your prefix such as Ms. or Dr.)
5. The current **Date** on page 5

Upload the completed PDF and click the Submit button.

After the UW DUA has been approved *for everyone on your team,* you will be able to participate in Subtasks B and C (Tracks 2B and 2C): Generalizability and Learning Transfer.
